PDA Surrey Return To Training Parent Consent (1)

With the current circumstances with Covid-19 we will be following the goverment guidelines along with the guidence from the FA. To return to training we will need consent from parents. We have created a consent form which covers all the information we require moving forward. This consent form is for Parents/Guardians to be fully aware they give permission for their child to attend group sessions during Covid-19

All players must arrive in training kit, have trainers and boots and they must bring a bottle of water.

There are no changing room facilities avaliable.
